MINIMUM ACCREDITATION REQUIREMENTS

  • Letter Request for Accreditation addressed to the Chairman, PNP Accreditation Committee Attention: Directorate for Research and Development (New or Renewal) with the following attachments:
    • Application Form
    • Information Sheet for President/General Manager/Proprietor
    • Information Sheet for Authorized Employee
    • Company Profile (Brief History of the Company)
  • Current Municipal/City Permit
  • Registration of Business Name with DTI (Single Proprietorship) or articles of incorporated and By-Laws from the SEC (Corporation).
  • Comparative Financial Statements for the last 2 years inclusive of a Balance Sheet and Income Statement with an Independent Auditor's Report and Income Tax Return.

    For newly organized business within the year, at least a Balance Sheet must be submitted with a letter/waiver addressed to the Secretariat explaining the reason for the non-submission of the Income Statement and Income Tax Return.
  • Directorate for Intelligence Clearance (to be secured from Rm. 207. Admin Building, NHQ PNP, Camp Crame)
  • Credit Limit Certificate issued by the Bank.
  • Certificate of Occupancy supported by a Title Certificate of Transfer or Certificate of Condominium Title (if applicant is the Owner of Office/Plant) or Contract of Lease (if applicant is Renting Office/Plant).
  • Sketch of the Company location and 3R size Photos showing the office/plant, number and street.

ADDITIONAL REQUIREMENTS

  • Sanitary Permit (dealing with Medicines, Catering, Food & Diagnostic Laboratories).
  • NTC License (for Communication Equipment and Repair.
  • ATO license (for Aircraft an Repair).
  • FED License (for Firearms, Firearms Spare Parts, Accessories Ammo, Pyrotechnics and Explosives).
  • BFAD license with Product Registration and DOH Accreditation (for Drugs/Food manufacturers).
  • Construction Services
    • Certificate of Good Standing issued by the Philippine Domestic Construction Board (PDCB)
    • Philippine Contractors Accreditations Board (PCAB) and Inter Agency Contractors. (IAC) License
    • Cash Flow
    • Organizational Chart with names of Engineers. Technical Staff with PRC number and other skilled personnel.
  • MARINA License (form Watercrafts, parts and Repair).
  • Certificate of Authority from the Insurance Commission-Department of Finance (for Insurance Services).
  • Custom Broker (CAB) (for Domestic Airfreight Forwarder).
  • Bureau of Product Standard Certificate (all products manufactured locally).
  • Dangerous Drug Board License (for Drug Testing Examination and Kits).
  • license of Chemist/Registered Medical Technologist/Pharmacist (for Drug Testing Examination).
  • Fertilizer and Pesticide Authority License from the Department of Agriculture (for Pest Control Services).
  • Distributor's Certificate (for all non-manufacturers of both local and imported products).
  • Previous PNP Certificate of Accreditation (Renewal).
  • Certificate of Good Standing issued by the End-User (Renewal). If no previous transactions was made, an affidavit must be submitted. For Drug Testing Centers (Renewal). Certificate of Good Standing must be secured from Civil Security Group (CSG).
  • Copy of other Accreditation Certificates (if any).
  • Authorization for an Employee with SSS Contribution Collection List.
  • Contract of Agency (Dealer/Supplier acting as Agent of Foreign Suppliers).

REMINDERS

  • All Documents must be NOTARIZED with a dry seal all the pages and their originals be presented to the Secretariat upon submission of application.
  • Only complete and clear applications/documents will be processed.
  • Applications must be submitted in triplicate sets bound in long folders containing a Table of Contents and tabs for distribution as follows:
    • Accreditation Committee thru the Secretariat at DRD
    • Directorate for Intelligence
    • Applicant
  • Deception in any form and providing incomplete information/data shall be grounds for denials processing of application.
  • Application for renewal of Accreditation must be submitted not less than forty-five (45) days before the Expiration Date.
